State Plan Personal Care and ADvantage Programs.  Clients may be eligible for personal care assistances and home making chores.  Client eligibility is determined by the OKDHS.  TEC, Inc. serves 25 Eastern Oklahoma Counties- Call today to check services in your county.  If you are interested in signing up for the program and you do not already have Medicaid, call us at 877-754-2593 and we will send a referral to the OKDHS.  If you already have a Medicaid number, all you need to do is contact your case worker at the local DHS and tell them you would like provider services through Traditions Extended Care, Inc. 

We are Licensed by the Long Term Care Authority, Oklahoma Department of Human Services & Oklahoma Health Care Authority to provide personal care and assistance with activities of daily living. We cover multiple counties - CALL TODAY TO SEE IF WE CAN ASSIST YOU IN YOUR HOME.
