PATIENT ADMISSION CRITERIA:

• Patient must be under a Physicians care.
• Home care services must be ordered by the patients physician.
• Patients physical and mental health status must require intermittent skilled care.
• Home care services must be furnished to patients in their place of residence.
• Patients must be basically home bound.

AS A CONSUMER OF HOME CARE SERVICES, YOU AND YOUR FAMILY HAVE THE RIGHT TO:

• Choose your homecare provider
• Not be solicited, coerced or harassed
• Be treated with dignity and respect
• Be fully informed of your rights prior to receiving services.
